Outdoor porcelain, patio tile lifting

A couple of years back I decided to lay a porcelain tile patio in my own garden and did lots of research and followed the steps that were online, and as far as I know I did everything correctly, but now I have found one of the tiles is loose and grout Has a few hairline cracks in different areas of the patio . I dug down 6 inches then used 100mm MOT type one subbase compacted with a whacker plate, then laid my tiles using a 50-70mm sand and cement mix (fully bedded not dot n dab) and used a slurry primeron the back of the tile. As I was told this was all I needed for adhesion and then used MAPEI ultracolour plus grout, which claims to be perfect for outdoor use. This past winter. The patio was covered in ice and there is where the problem started. What have I done incorrectly and how do I fix this problem without lifting the lot. Thanks in advance

you should have used reinforced mesh in the sand and cement bed as any little movement of the base can detach or crack the tile

With out looking I can not access properly, but at a guess, it sounds as if your tiles or grout are pourouce. It has taken in water. Once this freezes sbd expands it will push the tile from anything it's bedded too. Grout sealing is a must :)

During the winter some water got under the tiles and as soon as the water freezes it will expand causing the tiles to pop of, so maybe the grout hasn't been done properly.
