Are cavity closers only supposed to be installed at window and door openings in a wall?

Yes, modern cavity closers are insulated in the middle to prevent any cold spots .

yes cavity closures are installed to prevent cold spots close the cavity to then be foamed to create a seal

Cavity closure are used around windows, doors and any other openings on cavity walls, they are used to seal the cavity, plus as fire barrier around doors and windows, most of cavity closers this days are fire rated,
