What are the potential reasons why valves on a system do not hold?

Plumber needed to change actuator valve on the central heating system. He said to change it, he needed to drain down the system, which I understand. At some point doing something upstairs there was a leak and water flowed uncontrollably from tank down the outside of a pipe, which leads to the boiler. He said the leak was because the valves on system did not hold. What are the potential reasons why valve on system do not hold? Could there be another reason why water leaked? washers and seals can perish so although they appear to be shut they let by, gate valves jam and may seem to be shut but are not fully closed, all good plumbers know this,this is why you should have liability insurance.

Probably a gate valve which often don't make a water tight seal anyway. There could be many reasons, like water laying in pipes held back by an air bubble which then gives way.
