Raising a concrete floor level by 15-18mm

The entire downstairs of my house needs the floor levelling and raising by 15-18mm. Then karndean fitting on top. What is the best way to do this? The original floor in each room is uneven concrete at the moment. I also need to put down some sort of dpm? Can anyone help please?

Is use lecol pu280 2 coats as a Dpm lecol granular primer folled by lecol 7615 self levelling compound and add stone chipping to thicken the mix

You could use 18mm (hardwood) plywood sheets fixed down with hammer fixings and yes I would lay a dpm between the wood and concrete.
