I live in a mid terrace house and I am looking to extend to the rear by 3 metres, to increase the size of the kitchen, bathroom and the second bedroom. Would this require planning permission

yes -presume this is a 2 storey extension

Yes as it is two storey (or at least I gain that impression by your description). Permitted development restricts height which effectively allows single storey.
