Holes in grout and mildew under tiles - regrout and seal or tear out tiles and start over?

Our cubicle shower was installed a little less than 2 years ago - white tile with dark grout. Recently we noticed a mildew smell that would persist even after cleaning. Closer inspection revealed grout missing in several small sections (!) Which was not noticeable unless you shine a flashlight directly on it. The holes looked just like the dark grout. We pulled up one of the small floor tiles and noticed mildew or mold under it, in the space where the adhesive was not filled in completely. The builder suggests regrouting over all the grout and then sealing it. What to do?

That is a sign that you under tile ply is getting damp I will suggest to strip the area and re tile , you can always use same tiles if can be saved, grouting will just fix temporary but the wall or floor will get rotten if not changed .

I believe your just masking a problem if you just re grout as your sealing in whatever dampness (or worse)is under there, best to carefully investigate further by removing a few tiles in order to make a considered decision.

If its lasted two years of constant use,then it's ethier a leak from the shower valve behind the tiles or grout missing as mentioned.
Before ripping all out. Try the cheapest option. Regrout and seal. No point wasting money when this could prevent further issues.
Plus, no point scare mongering people, when simple solution could help
