Does the fireback on an open fire rest on the floor or on top of the hearth?

Just whacked out an old fireplace with a view to replacing it with a reclaimed open surround. The metal fireback currently sits on the floor (constructional hearth) with about 12mm clearance under the throat lintel. Is it normal for the fire back to go on the CH or the back of the decorative hearth? In either case I think the existing throat lintel will need moving up a brick course as 12mm won't be a very good hearth - but any advice welcome.
Are you a tradesperson and able to answer this question?

Its fine for the fireback to go directly onto the constructional hearth, however it is then more difficult to get the heath cut into the fireback! Hence most people will install it on top given the chance as its an easier install.
