What needs to be done to remove a "tarmacked yard", and cost per sqm?

Have back garden which has been tarmacked to just under damp cause. What needs to be considered, and idea of cost sqm. Dont know how deep, and there is a metal grid in the middle of it (water meter?). Idea is to replace with top soil and lay lawn.
Are you a tradesperson and able to answer this question?

Man and digger hire £150-£200 per day.
Labourer with wheelbarrow £50-£80 per day.
Grabber wagon to remove spoil £10-£15 per tonne.
Just a rough idea.. Make sure you hire someone insured and agree a detailed price as you don't know what you are going to run into befor digging. Depth/amount of waste to be taken away.
