Boiler fault hot and cold running water

Hi I have a potterton oil combi boiler about 9 years old it's been happening for some time but learnt to live with it when running hot water it gose cold after 2 mins the hot then cold and so on if turn the hot water tap on and have it very slow you do get hot water for a bit longer what do you think?
Are you a tradesperson and able to answer this question?

Hi.
Plate to plate heat exchanger clogged up. Boiler need new/cleaned one.
