Chimney leak mystery

Had a back tray of a chimney looked at and replaced twice bit water keeps seeping through. Could this be a condensation issue as the chimney is completely capped ?

It's not fitted properly , or you chimney stack could be leaking , ie through the flashing around it , or through the cap ?
