How to get rid of mother-of-thousands weeds from grass and borders!

Am at a loss as to how to get rid of this plant/weed which keeps popping up in different parts of the garden! Have tried weedkiller, a gas burner and slicing up chunks of soil and grass but it just won't be beaten! Any suggestions? It is a low growing ground covering plant with a lot of small round leaves with small white roots and even when you think you have got it all, it sprouts up again!
