Do I need false / suspended ceiling put in for spot lights in a flat

I want to have spot lights put in but I live in a flat. Is it possible to do this without putting in a false ceiling? How do I know if I will need a false ceiling put in? Will the electrician do this as part of the job? If I do need a false ceiling put in, will this significantly increase the price of the job?
Are you a tradesperson and able to answer this question?

Assuming it is a concrete ceiling, you would only be able to install lights by creating a void.
this must comply with building regs on construction and installation.
if it is a plaster boarded or plaster and lath ceiling then it maybe that you would be able to fit with some disturbance to the existing ceiling. You need to have a electrician look and make a informed decision.

Typically most flats will require access to the void above the ceiling to wire in for spotlights, if you're the top floor flat you may gain access through the loft space, if you've got someone living above you chances are they're not going to let you in to lift carpets / floorboards to achieve this (it may even be a concrete ceiling).
I would suggest installing a false ceiling, it will add cost as it will need to plastered afterwards but its probably less hassle than dealing with your neighbours / breaking fire barriers etc.
Some electricians will carry out or look after all the works for you.
